Recording a safety incident or near miss (HSSE)

Video walkthrough for this article has not been recorded yet — the written steps above are the full guide in the meantime.

HSSE is on by default for field seats — a safety module you can't write to from the field isn't much use, so this is one of the few things a field seat can fully create on its own.

Steps

  1. Open HSSE and start a new record.
  2. Pick the kind (incident, near miss, observation, toolbox talk, JHA, or inspection) and category (safety, health, security, environmental).
  3. Fill in date, time, location, who was involved, and a description.
  4. Set the severity, from first aid up to the most serious level.
  5. Attach whatever's relevant — a JHA document, a toolbox sign-in sheet photo, or evidence of a corrective action — using the same attachment button as everywhere else in Jig.
  6. If a corrective action is needed, assign it with a due date. As a field seat, you can close out corrective actions assigned to you; marking a record reportable or verifying a close-out is an office task.

One note: the reportable/notifiable flags in Jig are a reference, not a substitute for your own judgment on regulatory reporting — they don't walk you through an official determination.
